Your clinically supported Goal Plan can be matched with a dedicated Coach to strengthen follow-through, build consistency, and significantly increase the likelihood of success in the areas that matter most to you or your loved one’s future.

Regulation and Skills Support
As Nova Scotia transitions toward individualized funding, this program helps individuals and their support teams translate professional clinical guidance into clear, practical strategies that can be applied in everyday environments such as home, community programs, and residential supports.
Each participant receives a Goal Plan tailored to their unique needs and the needs of their support team. Plans are designed to be implemented through a Coach so strategies can be applied consistently across environments.
YLS coordinates with licensed clinicians such as psychologists, therapists, occupational therapists, and speech language pathologists. These clinicians provide therapeutic direction and approve strategies, while YLS Consultants and Coaches focus on helping participants and their supporters learn, apply, and maintain those strategies in everyday life until they have fully matured.
Our Process
STEP 1
Complete the Intake Application
Click and complete the "Inquire Here" button above. This provides baseline information about the individual accessing service, their support team, and the goals they or you hope to work toward. A Clinical Consultant will review the information and reach out to discuss your needs, programming & answer any questions.

Background and Clinical Review
A comprehensive background is gathered. This information is reviewed with the clinical team, and a Goal Plan is developed. The Clinical Consultant translates clinical guidance into clear, step-by-step strategies that can be implemented in everyday situations.
Program Selection and Design
The Clinical Consultant works in collaboration with the individual, their support team, and the Local Area Coordinator to review appropriate programming options across YLS. Together, the team determines what supports may be accessed through provincial funding, what may require private contribution, and how services can be combined to best meet the individual’s goals.
Step 3
STEP 1
Coaching and Implementation
A 1-1 Coach can be paired with the individual/care team to accompany the individual to programming and real-world application of the strategies outlined in the plan. The Clinical Team may highly recommend a Coach in more intensive situations where increased frequency with learning material will increase rates of success.
Step 4

Monitoring and Updates
Once services and funding are in place, the individual, caregiver, and support team meet regularly with the Clinical Consultant to review the Goal Plan and make adjustments as needed to reflect evolving wants and needs.
