CLINICAL CONSULTATION PROGRAM

Regulation and Skills Support
The Clinical Consultation Program at Yarmouth Life Skills (YLS) supports adults with intellectual and developmental disabilities in building practical regulation and life skills that improve daily wellness, independence, and participation in community life.
As Nova Scotia transitions toward individualized funding, this program helps individuals and their support teams translate professional clinical guidance into clear, practical strategies that can be applied in everyday environments such as home, community programs, and residential supports.
Each participant receives a Goal Plan tailored to their unique needs and the needs of their support team. Plans are designed to be realistic, accessible, and sustainable so strategies can be applied consistently across environments.
YLS coordinates with licensed clinicians such as psychologists, therapists, occupational therapists, and speech language pathologists. These clinicians provide therapeutic direction and approve strategies, while YLS Consultants focus on helping participants and their supporters learn, apply, and maintain those strategies in everyday life.

Our Process
STEP 1
Complete the Intake Application
Email us at ed@yarmouthlifeskills.com. We'll reach out with some basic questions about the individual, their support team, and the goals you hope to work toward. Once that information is gathered, a Clinical Consultant will review the information and reach out shortly to discuss your needs and answer any questions.​​

Background and Clinical Review
A comprehensive background is gathered, including the individual’s history, trauma history when appropriate, current supports, wellness indicators, and the values important to the individual and their family or support network. This information is reviewed alongside the participant’s clinical team to ensure strategies align with existing therapeutic recommendations.
Goal Plan Development
The Clinical Consultant works with the participant, their clinicians, and their support network to develop a personalized Goal Plan. The plan translates clinical guidance into clear, step-by-step strategies that can be implemented in everyday situations.

Step 3
STEP 1
Coaching and Implementation
A Coach is paired with the individual or family to support the learning and real-world application of the strategies outlined in the plan. Coaching helps ensure the strategies are practiced consistently across home and community environments.
Step 4
STEP 1
Monitoring and Updates
The Consultant tracks wellness indicators and works with the participant, their Coach, and their clinicians to adjust strategies when needed, ensuring the Goal Plan continues to reflect the individual’s goals and evolving needs.
